The Florida Department of Education, Office of Student Financial Assistance requests that participants of
scholarship and forgivable loan programs complete the appropriate form(s) to verify their program status.
Participants may reach the Office of Student Financial Assistance toll-free at 1-888-827-2004.
Participants of the programs listed below should use the following form(s) for status verification.
- Critical Teacher Shortage Forgivable Loan Program
- "Chappie" James Most Promising Teacher Scholarship Loan Program
- Masters' Fellowship Loan Program for Teachers
- Critical Teacher Shortage Scholarship Loan Program
- Occupational Therapist/Physical Therapist Scholarship Loan Program
- Status Verification Report (SVR): - (pdf) This form
notifies the Department of changes in the participant's name, address, telephone number, or projected graduation date.
- Certificate of Employment Service (CES): - (pdf) Teachers who are
completing one year of teaching service during the 2007-08 school year are required to report teaching service on a CES form
within 45 days after the end of each school year. The participant's superintendent or the
superintendent's designee (i.e., the school principal) must complete the CES form on or after the last
day of attendance by students in that district.
- Request for Deferment or Notice of Employment Service (DNES): - (pdf) Program participants
who are in "grace" status are not required to complete the DNES form.
- Program participants who are continuing their education on a full-time basis may request an
Educational Deferment (Section II).
- Program participants may also file for deferment based on inability to secure eligible
employment (Section III), temporary total disability (Section IV), or undue hardship (Section V).
- Program participants who have secured employment in a Florida publicly-funded school for the 2008-09
school year are required to complete the Notice of Employment Service (Section VI).
Submit requests for deferment 45 days prior to the deferment start date on the Request for Deferment or Notice of
Employment Service (DNES). Failure to complete and return the appropriate form could place the participant's account in
repayment status, which would then accrue interest in accordance with the terms of his/her promissory note.
Participants of the Paul Douglas Teacher Scholarship should use the following form(s) for status verification.
- Status Verification Report (SVR): - (pdf)
This form notifies the Department of changes in the participant's
name, address, telephone number, or projected graduation date.
- Certificate of Teaching Service (CTS): - (pdf)
Teachers who are completing one year of teaching service
during the 2007-08 school year are required to report teaching service on a CTS form within 45 days after the
end of each school year. The participant's superintendent or the superintendent's designee (i.e., the school principal)
must complete the CTS form on or after the last day of attendance by students in that district.
- Request for Deferment (RFD): - (pdf)
Program participants who are in "grace" status are not required to
complete the DNES form.
- Program participants who are continuing their education on a full-time basis may request an Educational
Deferment (Number 1).
- Program participants who are serving full-time active duty in the Armed Forces of the United States or
serving as a member of VISTA or the Peace Corps may request this deferment (Number 2).
- Program participants may also file for deferment based on temporary total disability (Number 3); needing to
care for a spouse, child, or parent (Number 4); inability to find full-time employment (Number 5); or undue
hardship (Number 6). Submit requests for deferment 45 days prior to the deferment start date on the RFD
form. Failure to complete and return the appropriate form could place your account in repayment status, which
would then accrue interest in accordance with the terms of the promissory note.
